New Patients

We are happy to register new NHS patients within our practice boundary area.

How to Register

IMPORTANT NOTICE -We will not accept incomplete Registration .  Patients must complete  the GMS1 form and New Patients questionnaires  and provide Proof of address and ID  . Failing to do this will result in you not being registered at the Practice .Any incomplete registration forms will be discarded  after 10days  .

To register with our practice, please follow the link below to complete the online registration form.  Adults/Children  Patient/Guardian  must complete  the GMS1 form and submit   it with  the Registration Questionnaire. You must be able to provide the Practice with one proof of face  ID and  one proof of address  .

Children Registration ,Parent /Guardian  must  complete the GMS1 form and submit it with the completed Baby and Child questionnaire .Please forward on the immunisation details   for children 0-6years old .If your child was born abroad you must supply the Practice  with the immunisation details  if unsure please contact the Practice.

After you have completed and submitted the GMS1 Registration form you must complete the downloadable New Patient Questionnaire  below, and attend the surgery within 7 days with these completed and two forms of identification. During Covid 19 Please email the New patient questionnaire to

Acceptable identification includes:

  • Passport
  • Birth Certificate
  • HC2 Certificate
  • Rough sleepers’ identity badge
  • Hostel Registration/mail forwarding letter.
Until we have seen your identification you will not be registered at the practice.
