Registering

If you have not previously been registered with a General Practitioner in this country and you are entitled to NHS treatment or if you are looking to register with a new surgery, you can pick up a registration form from the Surgery or you can download it on your computer by clicking the links below and bring the completed form into the surgery. Please bring in a proof of ID, and a proof of address although it  is not essential to bring a proof of address or proof of ID  but it would be useful

 

PLEASE NOTE ALL  PRINTED REGISTRATION FORMS MUST BE COMPLETED AND  HANDED BETWEEN 12noon to 5pm  INTO RECEPTION

Your medical card will be sent to you from the Primary Care Trust.

SUMMARY CARE RECORDS: Will allow Doctors at the Hospital to know what medications you are taking and if you have any allergies or bad reactions to medicines in order to treat you safely.

IF YOU WISH TO OPT IN / OPT OUT OF SUMMARY CARE RECORDS PLEASE ASK THE RECEPTION FOR AN OPT OUT FORM.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.Temporary patients services form

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

If you are Disable and need our assistance please ring the bell and a member of staff will come to assist you into the building .Our main entrance which is on the Kenton Road has a Wheel chair access , at the back of the Nash court builidng we have a car park, spaces are very limited ,however there is a designated disable car space to use. We advice patients to use Sainsburys Car park which allows 3 hours of FREE PARKING. Our second entrance  has a  lift facility  We allow Guidedogs for partially or non sighted patients.

The Practice offers  assistance in completing your Registration form .On Request we can print out information for you in LARGE  PRINT pleaes ask the Reception Staff .We  also offer support aid  to help with your consultations via British Sign language  / Interpreter

SMS TEXT MESSAGING SERVICE

The Practice is using SMS text messaging  service to communicate with their patients .It is your responsibility to provide the Practice with your upto date contact details  failing  to do so will result in delay in your treatment.   

  • The Doctors will be contacting patients regarding  Results  via SMS text messaging service ,if your result is NORMAL there is no need to contact the Practice
  • Referrals-  If you have been referred  by your doctor ,you will be sent a SMS text message  or a letter   regarding your unique password.

Patient Access On Line Services

Patients can now register with the PATIENT ACCESS  ON LINE SERVICE, you can now book /cancel your appointments, order prescriptions  and can also view your medical records  online. Once you have registered you would need to collect your ACCOUNT ID from your Medical centre, please bring one PHOTO ID
