Dear Patients,

ALWAYS CONTACT THE SURGERY BEFORE COMING INTO THE PRACTICE: All GP appointments are now predominantly telephone consultations unless otherwise advised. Nurse/phlebotomy/HCA appointment’s will continue to be face to face

As a Practice we  have been using Econsult online forms  for the past one year .You can submit  your Medical condition query, Prescription ,Medical certificate requests and other admin query via Econsult.  . When you send the forms to us via our website www.kentonbridgemedicalcentre.org.uk  they are then  clinically triaged by a clinician  .Please when calling the practice listen carefully to the message given on the answerphone before selecting to speak to our Receptions staff regarding this. We are still getting patients coming through on the phone demanding an immediate face to face appointment with a GP and when they are refused patients are being very rude and aggressive .

If there is a valid reason that you cannot complete an E Consult form online please advise our receptionist accordingly and in a polite and respectful manner so we can deal with the situation for you. All face to face appointments and home visits must be clinically agreed by a clinician first so again please do not demand this from a receptionist as they cannot do this for you. Equally, if having triaged your E Consult form a clinician deems your appointment to be routine and to be booked an appointment accordingly do not challenge the receptionist regarding this as they cannot overrule a clinician’s decision. We work hard to support our patients and appreciate your consideration and understanding at this busy time –

Our Practice adopts the Government’s  “Zero Tolerance Policy on Violence and Aggression”, so please do not shout or swear at our Reception staff as they are only doing their job .

Face coverings: Message from North West London CCG – As you know, this change comes into effect on Monday, 19 July 2021. We have now received clear guidance from Public Health England, making clear that this change does not apply to healthcare settings. Current infection prevention and control guidelines (and hospital visiting guidance) remain in place for all staff and visitors. That means NHS visitor guidance will stay in place across all health services, including hospitals, GP practices, dental practices, optometrists and pharmacies, to ensure patients and staff and yourselves are protected. Staff, patients and visitors will also be expected to continue to follow social distancing rules when visiting any care setting as well as using face coverings, mask and other personal protection equipment. Please register for PATIENTS ACCESS ONLINE SERVICE to book/cancel appointments,(on hold at the moment) view your medical records and order your prescriptions on line.

Medical RecordsThe Data Protection Act gives every living person authorisation and the right to apply for access to their health records.

Please give all your previous prescription requests to the reception staff .When registering children between the ages 0-6yrs we require immunisation details .

Completed Registration forms with all the relevant documents must be handed between 10am to 5pm Monday to Friday.

Please note incomplete registration forms will delay your process in registering with the practice.

Please remember as a GP practice we cannot take the following over the phone: Repeat prescription requests– To be done via Econsult /Patient access or in writing

Change of medication requests – to be sent via Econsult or in writing

Change of address/Name information –to be sent via ourwebsite: or by coming to the practice to complete a change of address form

Insurance/Solicitors requests  letters – to be sent via email/in writing

Requests for Private referral letters – to be sent via email/ in writing

Please note for some administration letters there will be a fee ,please enquire about this from our Admin Team

Sick note requests – to be sent via Econsult /in writing

Please do NOT ask your GP to treat your Dental conditions, or refer you to Hospital for dental problems. They are not trained Dentists and their Medical Indemnity does not cover them for dealing with Dental problems. Please visit your own Dentist for the same. If you do not have a Dentist, please call 111 for a list of NHS Dentists in your area or visit https://www.nhs.uk/service-search/find-adentist.

Regarding certificates or evidence about your COVID vaccination status: We are unable to issue this certificate at the practice. Please see https://www.gov.uk/guidance/demonstrating-your-covid-19- vaccination-status-when-travelling-abroad for further information. Proof of your vaccination status will be available in the NHS app from 17th May 2021,
